Collaborates with multiple instructional team(s) that may be at multiple sites and districts, to support student achieving academic success in accordance with district policies. Duties and Responsibilities: * Assesses student needs and environmental barriers and identifies modifications and adaptations to best deliver instructional activities to actively engage students in meaningful learning experiences. Monitors students in the use of learning materials and resources for use in educational activities. * Plans and prepares materials and adaptations for appropriate subject and grade/age level and ensures maximum independence allowing for continuous student growth and achievement. Evaluates, selects, and modifies instructional resources to meet the needs of the students with varying backgrounds, learning styles, and special needs. * Effectively communicates, collaborates and consults with families, other classroom and special education teachers, and specialized service providers in designing and implementing IEP/IFSP plans for students. Participates in evaluations or re-evaluations as the teacher licensed in the area of disability. Establishes and maintains positive relationships with students, parents and staff. * Provides professional development to building staff. Conducts in-services for school personnel on topics related to disability and licensure area. Serves as a resource for campus administration and staff. * Manages student behavior utilizing proactive, positive interventions and implements appropriate behavioral management plans and crisis intervention strategies, as appropriate. Ensures due process rights of students. * Maintains accurate and complete records of students' progress and development including implementing plans for special education groups. May develop and write annual IEP/IFSP's, progress reports and three-year reassessments and complies with due process requirements. * Assesses changing student needs, offers plans for improvement, and develops new resources within District guidelines. Keeps current with developments in subject area, teaching resources and methods and make relevant changes to instructional plans and activities. * Participates in department/school meetings and parent meetings. * Performs other job-related duties as assigned. * Minimum Requirements: Requires a minimum of a bachelor's degree and licensed or able to obtain a license from the MN Department of Education in Physical & Health Disabilities (Pre K-12).  Job Summary:
