Serves as a member of a multidisciplinary team to assess and/or reassess students referred for special education services. 2. Prepares and implements appropriate service plans for special education students, refers child/family to community agencies as needed, documents progress in the Individual Education Plan (IEP), attends IEP meetings. 3. Serve as the building contact and support for students with 504 plans; coordinating 504 referrals, evaluation, collaboration, planning and documentation. 4. Provides individual and group support to help students resolve and/or cope with personal, emotional and social problems that interfere with their educational experience. 5. Provides services to parents as an integral part of the task of helping students; to gain their parental participation in resolving their child's problems. 6. Assists families to better understand the school and its' programs and to strengthen the communication link between school and home. 7. Assists teachers and school administration through identification of special needs a child may have in school. 8. Serve as a liaison between home, school community and other agencies through maintenance of positive relationships. 9. Provides crisis intervention services. 10. Participates in faculty meetings, special education meetings and in-service training. 11. Fosters social work development through participation in workshops and conferences. 12. Performs specific duties related to school social work which may be assigned by the Executive Director of Student Support Services. 13. Performs other duties of a similar nature or level. Specific Responsibilities: 1. Provide assessments which involve preparation of social and developmental histories. 2. Other tasks include administering social interaction instruments, interviewing parents, child and staff, observation of child in school, collection of agencies reports, and review of school information. 3. As the building contact for Section 504, will coordinate 504 referrals, evaluation, collaboration, planning and documentation. Knowledge and Skills: 1. Has demonstrated knowledge in using a variety of behavior management strategies. 2. Demonstrated ability to apply appropriate skills in performing daily routines. 3. Demonstrated ability to operate a computer, software and necessary technology. 4. Ability to develop effective and positive working relationships with students, staff, administrators, parents, and the community. 5. Show initiative and function as a self-starter. 6. Strong communication skills and interpersonal skills as applied to interaction with coworkers, supervisor, the general public, etc. sufficient to exchange or convey information and to receive work direction.
