POSITION TITLE: School Social Worker DIRECT SUPERVISOR: Special Ed Coordinator POSITION STATUS: Exempt CORE HOURS: Monday &#8211; Friday, 7:30 am &#8211; 4:00 pm DUTY DAYS: TBD ______________________________________________________________________ POSITION PURPOSE: The School Social Worker will be working with students in two distinct programs on campus; a transitions program and and mental health program serving secondary school aged students.The School Social Worker will assess, evaluate and determine the social, emotional, behavior, mental health needs needs of students within these programs, provide direct and indirect services, collaborate with interdisciplinary team, and act as a liaison between school, home and community social systems. ESSENTIAL FUNCTIONS: * Assess and evaluate students to determine individual needs based on social, emotional or behavioral concerns. * Develop and document present levels of performance, IEP goals and objectives based on outcomes of assessment * Act as the lead for developing student programs related to mental health and other social, emotional, or behavioral needs. * Provide direct and indirect service based on outcome of assessment. * Counsel individual students regarding issues which interfere with adjustment and/or performance within the educational setting. * Provide consultation on classroom management procedures and organizational structure. * Provide appropriate outreach and act as a liaison between school, home and community. * Effectively supports students and families in navigating, understanding, and applying for community services and supports * Provide crisis intervention services as appropriate and as requested by administrative staff (suicide assessments, conflict resolution, behavior management). * Maintain accurate case records on all students regarding referral, evaluation, and therapy services. * Focus on PBIS campaign including mediation, character education and general awareness of bullying behaviors. * Participate in IEP meetings and student case conferences, as needed. * Maintain positive, professional relationships and use appropriate verbal, nonverbal and written communication with all individuals * Provide professional development presentations/training to staff on relevant topics * Possible case management * Regular, prompt attendance OTHER POSITION RESPONSIBILITIES: * Participate in professional growth activities * Substitute staff absences, as needed * Perform other job-related duties as assigned * Lead small group instruction on topics related to transition and young adulthood PHYSICAL DEMANDS The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job.
