School Health Coordinator

 

Hiawatha Academies is a growing network of high-performing charter schools in Minneapolis, MN. We are driven by our mission that "All Hiawatha Academies scholars will be empowered with the knowledge, character, and leadership skills to graduate from college and serve the common good."

 

We have demonstrated success in student achievement, and recently received the honor of the Minnesota’s Future Award from the MN Business Partnership. Our vision is to expand our impact by growing to 5 schools by 2020. At scale, we will be putting 5% of all school-age scholars in Minneapolis on the path to and through college.

Our Network is seeking a School Health Coordinator to join our team with a preferred start date as soon as possible.

 

Job Description

 

The School Health Coordinator is responsible for the management of the school-based health program. The School Health Coordinator will collaborate with all school staff in maintaining a high level of wellness in the entire school population.  S/he will facilitate the District's compliance with mandatory responsibilities related to health. The School Health Coordinator will report directly to the Director of Student Support Services to ensure that effective programmatic supports are provided to each campus.

Responsibilities

 

·         Manages the process of maintaining and updating student health records

·         Coordinates with campus staff to collect, record, and report immunizations

·         Coordinates annual vision and hearing screenings with campus staff

·         Performs health screenings and assessments for students and students referred for suspected health problems

·         Coordinator accommodations plans under Section 504

·         Obtains a health history

·         Develop, plan and implement network health policies and procedures

·         Screens and evaluates findings as indicated during screenings

·         Supervises prescribed medical treatments and medication administration according to School Board policy

·         Provides assessment, planning, intervention, evaluation, management, and referral for appropriate medical care of students with acute/chronic health conditions and in emergency situations

·         Coordinates the provision of first aid supplies as needed

·         Assists in the development and implementation of a students’ 504 plans, when necessary

·         Interprets the health status of students to parents and school personnel

·         Provides ongoing health counseling with students, parents, school personnel or health agencies

·         Utilizes existing health resources to provide appropriate care of students

·         Maintains, evaluates and interprets cumulative health data to accommodate individual needs

·         Coordinates school and community health activities and serves as a liaison health professional between the home, school and community

·         Acts as a resource person in promoting health for all campuses

·         Acts as a resource for infection control for all campus

·         Participate as a member of the Safety Committee to plan and implement emergency response protocols

·         Perform other duties as required and assigned

Hiawatha Employment Logistics

·         This role would be a part of the Network Support Team

·         The work day for most Network Support Team members is from 7:15 am-4:30 pm, with some flexibility according to role and circumstance

·         Regular weekly presence at all Hiawatha campuses (currently 3 Minneapolis schools) will be required; must provide own transportation

 

Skills & Qualifications

·         Minnesota School Nurse License or meets qualifications to obtain license through Minnesota Department of Education (MDE)

·         Registered Nurse in Minnesota

·         Experience working with students of color and / or historically marginalized communities

·         Must be certified in CPR

·         A passion for and commitment to being part of a team-oriented, mission-driven organizational culture

·         Excellent communication and interpersonal skills and ability to build trust with colleagues, students, and families

·         Must have a growth mindset

·         Awareness of self and the ability to understand, appreciate, and interact within different cultural contexts

Highly Preferred:

·         Bilingual or multilingual (Oromo, Amharic, Spanish, Somali, and / or Hmong)
