The successful candidate will possess the following attributes:
* Experience with case management
* Collaboration and communication skills
* Knowledge of due process including the referral and testing process
* Experience teaching social skills to individuals and small groups
* Understanding and experience in utilizing instructional best practices to engage elementary students in academic, social, and emotional learning
* Understanding and experience in assessment and data driven decision-making
*Current Minnesota teaching license in ECSE
*Provide services to families and children from birth to age 3 and 3-5 if needed: developmental evaluation, child and family assessment, family-centered services, service coordination, direct services and collaborative consultation with staff, parents, agency personnel, and administrators.
*Supports the referral process including initial contact with families.
*Assists families in understanding child development, identifying developmentally appropriate expectations, understating the etiology and physiology of the child''s delay, disability and/or diagnosed condition. Supports the family by relating this information to the needs of the child, and by providing strategies and resources to assist the family in addressing the child''s needs.
*Provides periodic home visits to the child and family providing support to enhance parent-child interactions and developmental guidance. Supports the family in providing an appropriate environment and activities and strategies to enhance their child''s development.
*Provides developmental supports, services, and on-going assessments in natural environments for children, age 0-6, and their families according to their Individualized Plans.
*Makes referrals to and coordinates with appropriate community resources and existing at-risk/special needs services to assist and support child and family needs.
*Coordinates services for eligible Part C/Part B children and their families by developing and maintaining the Individualized Plans.
Will have access to a special education case facilitator who coordinates Help Me Grow referrals, intakes and assists with assessments.
Full-time, beginning the 2026-2027 school year.
