Description: 1.0 FTE Social Worker at Fridley Middle School for the 2023-2024 school year. (This is a permanent position extending past the 2023-2024 school year). Job Summary: Performs professional work within district multi-tiered system of support through obtaining and analyzing data to determine social-emotional needs of students. Serves as a liaison/link between students and federal, state and local social and community support programs. Serves as building homeless liaison. Work is performed under the supervision of the Building Principal and Director of Special Services. Provide basic and comprehensive services to individual students and families which would include: 1. Assist in diagnosing students&#8217; needs; 2. Provides crisis intervention and prevention for families and students; 3. Collaborates with school nurse, community agencies and industry professionals to meet individual student needs; 4. Conducts interviews to obtain student social history; 5. Refers students to and serves as a liaison with outside agencies; 6. Conducts home visits when necessary; 7. Assist students with social-emotional learning.
