Live Well at Home Care Coordinator

Responsible to screen and assess frail older adults, develop an action plan to reduce risk, provide community supports, and provide home aids and minor/major home repairs to improve the physical safety of the elder’s home

 

Duties and Responsibilities

·         Conduct personal health screenings and home safety assessments

·         Develop an action plan to reduce risk and provide supports and referrals to appropriate resources
